Permit Application Instructions

You must fully complete the four-page application.

The Application form in PDF format:

1. Please print the form from the web site. You cannot fill this PDF from your computer.
2. Please “print legibly” all required information and sign the form on both pages.
3. Complete the Credit Card information. We only accept VISA or MASTERCARD.
4. Fax the form to (705) 887-2199. Only the completed four pages are required to be sent.
5. Or Mail the completed form for a cheque payment.

General Information;

Your permit will be forwarded to you with 48 hours.
You must complete all information and V.I.N.’s.
You must complete separate forms for each registered machine.
